Documents necessary for death registration:

Application;

Identity document of the applicant (identification card of a citizen or passport);

Identity document of the deceased person (identification card of a citizen or a passport);

One of the following documents certifying the fact of decease:

Document on death issued by a health institution;

Court decision on establishment of the fact of death;

Resolution of a civilian registration authority on establishment of the fact of death;

Notice of the administration of the penitentiary institution on the fact of the servitor;

 Death certificate of persons, repressed according to the resolution of injudicial (ministries of internal affairs, state security and defense) and judicial authorities;

Report, made by a public official of self-government, authorized by the executive authority of the local self-government;

Registration of death of the persons, lost as a result of a natural disaster or a catastrophe can be made on the basis of the respective legislative act of the President of Georgia;

Registration of death of servicemen related to hostility is made according to the military department of the local self-government, where they were registered, according to the headquarters of the military units, where the serviceman served military obligations.

The certificate is issued free of charge the day of submission of the application and necessary documents, stipulated in the law. The application on death registration may be submitted according to the last place of registration of the deceased or in the respective territorial office of the Civil Registry Agency according to the location of the death registration document issue.

• Documents to be submitted for reissue of acts of civil status;
• Documents to be submitted to make corrections and alternations in acts of civil status;
• Categories of persons, exempted from payment of state duty for registration of acts of civil status concerning the cases, realized by territorial offices of  the Civil Registry Agency;
• Documents to be submitted to take certificate on absence of circumstances, impedimental to marriage;
