1. If filiation is made according to the mutual consent of both parents. Parents should submit the following documents to take child's birth certificate:
- Joint written application of parents;
- Child birth certificate;
- Documents verifying parents' identity (identification card of a citizen of passport).
The birth certificate is issued free of charge the day of submission of the application and the documents, stipulated in the law. The application on filiation registration can be submitted to both or one of the parents' registration places and according to the place of making of the judicial decision on filiation in the respective territorial office of the Civilian Registry Agency.
2. Father of the child should submit the following documents to take the filiation certificate on the basis of father's application:
- Written application of the father;
- Father's identity documents (identification card or passport);
- Child's birth certificate;
- Mother's death certificate or a judicial decision on acknowledgement of mother as missing, or a judicial decision on acknowledgement mother to be incapable, or a judicial resolution on deprivation of mother's parental rights, or the respective document on failure to establish mother's location area;
- A judicial decision on filiation.
The birth certificate is issued free of charge the day of submission of the application and the documents, stipulated in the law. The application on filiation can be submitted to both or one of the parents' registration places and according to the place of making of the judicial decision on filiation in the respective territorial office of the Civilian Registry Agency.
3. The other parent should submit the following documents to establish and take filiation certificate, if filiation is realized against father's will, father is deceased or acknowledged as missing:
a) Filiation realized against father's will:
- Written agreement of a parent;
- Birth certificate of the child;
- Identity document of the parent (identification card or passport);
- Judicial decision on filiation;
b) If father is deceased or acknowledged as missing:
- Written agreement of a parent;
- Birth certificate of the child;
- Identity document of the parent (identification card or passport);
- Judicial decision on acknowledgement of the person to be missing or father's death certificate;
- Judicial decision on filiation;
